CHRODIS PLUS pilot projects - Implementing good practices for chronic diseases

The Joint Action CHRODIS PLUS is a three-year initiative (2017-2020) funded by the European Commission and other organisations representing 21 European countries. This initiative's overall goal is to raise awareness and acceptance amongst decision-makers on improved actions to prevent and manage chronic diseases.

To test tools and best practices in addressing chronic diseases, CHRODIS PLUS has implemented 21 pilot projects - one for each of the involved countries - aimed to promote long-term sustainability and integrate good practices into policy. As a result, 16 policy dialogues have been organised, bringing together 50 partners from all over Europe.
